Recordation of Changes

The Required Documents for Recordal of Changes (Owner’s Name and/or Address) to Registered Trademarks

For recording a Change of  Name:

1. A notarized OR an originally-certified true copy of the document evidencing the name change;
2. A simply signed Power of Attorney.

Note:

• All Trademark Registrations in the name of the same should be recorded simultaneously.

For recording a Change of Address:

1. A simply signed Power of Attorney.
